Ways to Order a Certificate

You can order a State of Colorado birth certificate or a Denver County death certificate online, by mail, or in-person (walk-ins are always welcome). The cost for a first copy of a certificate is $20. Additional copies are $13. Expedited shipping is available via FedEx for $25. You will receive a response to your order within 30 days.

If you would like to order a birth or death certificate in person, please visit us at 120 W. 5th Ave., Denver, CO 80204. Our hours are Monday-Friday 8 a.m.-4 p.m. We are closed all City and County of Denver holidays.

Official copies can be obtained on site, the same day, if regulated requirements are met.

Step 2.Complete the Application

You will be asked to complete a form, and to present your current driver's license, state identification, passport or other form of accepted identification.

Step 3.Make Your Payment

Payment will be processed at the office. We accept cash, checks and major credit cards.

By Mail

To request a birth or death certificate by mail, please fill out the birth certificate request form, or death certificate request form, and mail the signed application, fee (check or money order; do not send cash) and photocopies of the appropriate proof of identity or relationship to our office at:

Who can order a birth certificate?

The person named on the birth record, with approved forms of identification.

Members of the immediate family, with required proof of relationship. To prove your relationship, we accept:

  • Birth certificates
  • Marriage certificates
  • Court-approved guardianship paperwork

Who can order a death certificate?

Death certificates may be issue to a current spouse, parent, shared parentage, siblings/half siblings, children, or a legal representative of those named.

What forms of payment are accepted?

We accept VISA, MasterCard, American Express, Discover, checks, and cash as forms of payment.

What forms of identification are accepted?

A valid driver license, state issued identification or passport that are not expired is required. If you do not have the above identification, please refer to this approved identity verification documents listing.

For who can I purchase a birth certificate?

You may purchase your own, or one for your:

  • Mother
  • Father
  • Children
  • Spouse
  • Siblings (a birth certificate proving one common parent required)
  • Grandparents
  • Grandchildren

I was not born in Colorado. Can I get a birth certificate from your office?

Unfortunately, no. You must contact the vital records office in the state in which you were born.

How do I make corrections to my child's birth certificate?

If the child is under age 1

To request a correction or to make a change to a birth certificate, you must submit the below items to the State of Colorado Vital Records Office:

If the child is over age 1

To change the name on a birth certificate for an individual older than age 1 please reach out to the county court office to obtain a certified copy of a legal name change order BEFORE submitting the birth certificate correction form.

Acknowledgement of Parentage Information

To add a father or mother to a birth certificate when a second parent is currently not listed, complete and submit the Voluntary Acknowledgement of Parentage form to the State of Colorado.

 

Other Important Information

Changes to identifying options on birth certificates

On Dec. 19, 2018, the Colorado Department of Public Health and Environment's (CDPHE) Board voted to allow more identifying options for birth certificates. There will now be four options: male, female, X and intersex. To make changes on a birth certificate, contact CDPHE

Birth certificates for Colorado adult adoptees

The Colorado Department of Public Health and Environment (CDPHE) began processing requests for original birth certificates for Colorado adult adoptees on Jan. 4, 2016.
